To request an individual, children, couples, marriage or family therapy appointment or to obtain additional information about any of these counseling services, please fill out the form below or give us a call. This link will not schedule an automatic appointment but will give us an idea of your availability so that we can set you up with a therapist that will work for you.

 You may also inquire about our Intensive Outpatient Program and we will call you to provide you with additional information.
